VERIFIER APPLICATION FORM
This form must be completed by individuals who have already received Assessor training and would like to be trained as an Internal or External Verifier to work with CVQ Programmes at Approved Training Centres.

To apply for a Prior Learning Assessment, please download, complete and submit this form to the SSDA/NQD. Applicants should have a minimum of five years experience in their industry.

This form should be completed by individuals who wish to complete the Assessor Training Programme to become an Assessor in their Occupational area. Applicants must have at least an Associate Degree in the area in which they wish to assess.
